This is not an online entry form. Print and complete a copy of this form and the “Competition Menu” (or the PDF version of each) for each trailer submitted (maximum of 2 per team/filmmaker). Send along with $10/trailer entry fee (cash or check) and your DVD to:

no later than July 2, 2009 Trailer Smackdown deadline extended until Monday, July 6th! This gives you another weekend to perfect your masterpieces of trailer-goodness.
